End of Life
The goal of End of Life Care is the achievement of the best quality of life for service users, carers and their families. Carers Trust Mid Yorkshire will work in partnership with others to provide a range of services to support people with advanced, progressive, incurable illnesses to live as well as possible, promoting independence for as long as possible. Suitably trained staff will provide practical, consistent, flexible support to service users, their carers and their families, to meet individual needs, to enable those who choose to die in their own homes to do so.
- Help avoid unnecessary and unwanted admissions to hospital or residential care for terminally ill people who wish to remain at home
- Foster in those for whom we care, a sense of belonging to the wider community, increasing choice and enhancing their quality of life
- Promoting the involvement of the person with care needs and the carer in the care plan
We provide support to an individual in all aspects of their personal care needs which will be identified within their care plan.
We can provide practical support to carers and their families including shopping, light domestic duties and preparation of meals. We aim to relieve some of the pressures faced by carers at a very difficult time.
This is an important role for our staff. They can offer a listening ear or a shoulder when needed, to the person with care needs and/or their carer. They can also signpost to other local resources or liaise with health care professionals to ensure there is effective continuation of care.
